The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a New Patient to our office, we encourage you to call or fill out our Contact Us form, so we can send you a link that includes all New Patient Forms, that will be uploaded directly into your patient account. If you have a preference for a paper copy of our New Patient forms, please feel free to stop in prior to your scheduled appointment to request them. Having your forms filled out allows us to attend to your medical needs and questions, more quickly, than completing them upon your arrival. Thank you and please call our office if you have any questions at all. 

Dental Insurance

We accept all insurances regardless if you are in or out of network. Feel free to call your dental insurance
provider to find out if you are considered in or out of network.

Payment Policy

We currently accept all major credit cards (Visa, Mastercard, American Express, etc.)
In addition to major credit cards, we do accept cash, check, and Care Credit. We currently do not offer in house financing at this time. Our financial practice policy is payment due at time of service.

We offer a complimentary discount for the following:

• 5% discount for payment in full for those without insurance at time of service
• 10% discount for seniors (65 and older) for payment in full for those without insurance at time of service
